We can provide bespoke packages of care to individuals within Shropshire and the surrounding areas and are experts at working alongside people with complex health and social care needs. We provide transition work for those out of area who are wishing to move back and those who have experienced longer stay hospital admissions and accept Direct Payments as well as Local Authority and Continuing Healthcare funding.
We work alongside housing providers to ensure appropriate accommodation alongside individual packages of care to ensure that housing and support services are specific to meet individual needs.
When supporting a service user, we will send regular updates and reviews to professionals and family members and have positive working relationships with our multi-disciplinary colleagues.

Once we have received a referral, we will call you for discussion and to arrange an assessment at a convenient time. We have a wealth of experience assessing people who might have complex needs and undertaking appropriate risk assessments.
